There isn't a straightforward translation of "scan" in European Portuguese, "escanear" isn't a word that is really used in Portugal. The translation for "scan" is therefore very much dependent on context. And in this case it's scanning a document, which is referred to as "digitalizar". So that's what I went with. Also, machine-readable is most often called optical reading, so I changed that as well.
