1.Both of the previous suggestions say " Is it true that you want to leave?" instead of asking if one is sure.
2. There's a typo "Haqiqatan". It is "haqiqatdan" (haqiqat (arabic) "truth" + suffix -dan, even speaking it would sound with a little abruption like "haqiqat'tan.

Both can be used. The suggested translation is just a preferential change. There is no typo in "Haqiqatan". Correct spelling of the word can be checked here: https://savodxon.uz/izoh?haqiqatan

Haqiqatan ham %@ni tark etmoqchimisiz?ishga ishonchingiz komilmi? - "ishonchingiz komillmi" is not spoken uzbek, and not so popular, but I belive this is exactly asking "are you sure you want to leave"
